Our registration process is robust but simple. You can download a copy of our registration form and equal opportunities form which we would be grateful if you could complete and return. You would also need to send us copies of the following (by fax, post or e-mail) to register;

 

  • GMC certificate
  • Birth Certificate or Passport
  • PMETB certificate
  • MDU insurance
  • Performer’s list letter
  • Up to date CV
  • Hepatitis B Certificate
  • CRB Enhanced Disclosure
  • We also require evidence of your last appraisal.
